New 2009-2010 Membership Registration Information

Membership registration will be available soon,
but all players must have a waiver on file at Premier.

Beginning October 1, 2009, in order to meet current insurance requirements, all players at Premier Volleyball Academy will be required to register and become a member of the Academy for the 2009-2010.  Membership is free to players if completed online in advance of the activity/event being held at Premier.  If members do not register online and instead register in person, the charge is $5.00.

All players regardless of age or activity must be a current member in order to participate on the courts at Premier. Players that show up in person without having an online registration completed will be asked to register in person and be charged $5.00 for the membership to be completed and imputed by hand by the staff.  Memberships are good for all activities at Premier Academy for the 2009-2010 season including Adult Leagues, Adult Open Gym, Glow Volleyball, Adult Tournaments, Juniors Open Gym, Premier Club Teams, M-Juniors Club Teams, Premier Academy Camps and Clinics, and all special events. 

Players only need to register one time for the entire season regardless of how many activities/events the player participates in.  Adults that participate in adult leagues or open gym should keep track of their membership number in order to sign in at the door or with the official.

 

Waiver and Release of Liability

In addition to the online registration, each member must have a completed and signed waiver and release of liability form on file at Premier Activity.  Memberships are not complete without a signed waiver and players cannot participate on the courts without a waiver on file.  This is for all activities at Premier Academy for the 2009-2010 season including Adult Leagues, Adult Open Gym, Glow Volleyball, Adult Tournaments, Juniors Open Gym, Premier Club Teams, M-Juniors Club Teams, Premier Academy Camps and Clinics, and all special events.  Players can mail in the waiver or turn it in on site, as long as it is on file prior to participation.

**As we begin to merge our current web site with our new registration system, please use the current registration forms and method of payment options listed now with the activity you are registering for.  Players that are registering now prior to our new system being active will not receive a membership number at this time, but you will be registered for your activity with no problem.  Once our new system is up and live (Approximately October 1st), we will contact you to come back and register with the new system in order to generate your membership number and create your player account in our system.  Players will still need a Waiver and Release of Liability Form on file beginning September 1st, 2009 in order to participate on the courts in any activity.  This form can be found on our web site with your registration for any activity.  We thank you for your patience while we merge into this new system.
